Incident / Accident Reporting
Free up the time of your supervisors and managers by letting our office complete an incident report for any incident or accident including self-care, first aid, medical only, and lost time claims.

- Supervisor or injured worker contacts our office to initiate report. QR code provided to initiate call to our office.
- Recorded statement of injured worker, witness/co-worker, manager/supervisor. Recorded statements preserve evidence.
- Report includes statements, description of how incident occurred, cause of incident, Equipment/PPE/chemicals used at time of incident, nature of injury, list of witnesses or co-workers, date of knowledge, work status, employee injury history, how future incident can be prevented.
- Complete Incident/Injury Report on date reported.
- Complete state forms: 5020 and DWC-1 for California.
- Submit report/state forms to employer. As directed by employer: upload to carrier, third party administrator and broker.

- Contact injured worker on date of medical appointment to obtain current status, any work restrictions, next appointment date, and discharge status.
- Obtain medical reports from clinic and provide to employer and adjuster.
- Provide billing information and claim number to industrial clinic.
- Discuss with employer work restrictions and modified duties availability.
- Draft offer of modified duties letter.
- Provide adjuster status regarding modified duties.
- Obtain/provide documents requested by adjuster.
